How to build a deployment health & risk dashboard

What is deployment risk?

Deployment risk is the chance that the next deployment causes a user-impacting failure. It is not a number you can read off a pipeline, because it comes from three separate signals that only mean something together.

  • Pipeline reliability: How often deployments complete without failing.
  • Change outcome: How often a deployment that did succeed still caused an incident.
  • Standing risk: What unresolved high-priority work is already open in the environment.

The deployment risk tile combines all three into a single score, which is why it comes last.

Data sources to use

Depending on how your teams operate and the technologies they use, you can draw from multiple data sources to build this dashboard.

In this example, we’ll use:

  • Azure DevOps to capture operational signals.
  • Jira to return the incident tickets raised since the deployment. If you track incidents somewhere else, such as Jira Service Management or ServiceNow, the same tile works against a data stream from that source instead.

This tile provides a high-level health signal that summarizes the current state of deployments. By combining multiple indicators into green, yellow, or red blocks, it gives teams an immediate sense of whether the environment is stable or carrying elevated risk.

Deployment frequency over time

This tile shows when deployments have occurred and how frequently changes are being pushed. It helps teams understand deployment cadence and provides context when investigating incidents or regressions that may align with recent releases.

Change failure rate

This tile models change failure rate by correlating successful build runs with incident-level bugs. Successful builds represent changes that were eligible to reach production, while incident-priority bugs represent negative, user-impacting outcomes.

This tile highlights high-priority issues and bugs that are still open while deployments continue. It surfaces situations where teams may be adding change on top of known risk, increasing the likelihood of compounded failures.

Deployment risk

Risk alerts bring everything together by flagging conditions that indicate elevated deployment risk. These alerts help you move from passive observation to action, notifying when multiple risk factors align and it may be time to pause or reassess further changes.
